Memory Care

Our memory care neighborhoods are for seniors with memory impairments such as Alzheimer’s and dementia. Along the Journey is Priority Life Care’s Memory Care programming for overall wellness. This person-centered care program helps families and caregivers develop relationships and enrich the lives of our residents in the memory care neighborhood.

Person-centered care is essential to dementia care and the underlying philosophy of the 2018 Alzheimer’s Association Dementia Care Practice Recommendations. Person-centered care helps build a mutually recognizing, respectful, and trusting relationship between caregivers and residents’ loved ones. The community provides consistent caregivers to support a resident’s sense of self. We also provide individualized and engaging activities, and offer guidance and additional support systems to residents’ family members.

Potential residents are assessed before admission into the memory care neighborhood, and regularly thereafter. Our care plans are unique to each individual and flexible enough to accommodate each resident’s feelings and needs. The family will also complete a “Familiar Routine and Life Story.” This will help the care staff optimize residents’ independence and feeling of self through building on strengths, interests, and abilities. The more we can get to know seniors prior to move-in, the better we can serve them.

Each time a new neighbor moves in, we focus on their transition and acclimation to the neighborhood. Our goals of Along the Journey are to encourage overall wellness of our neighbors through:

  • Creating a safe and comfortable environment for residents
  • Consistent, well-trained caregivers specific to dementia care
  • Providing care based on resident’s abilities and needs, with respect to the individual’s culture and preferences
  • Achieving highest quality of life possible through cueing and assistance
  • Being a resource and provide support to families, staff, and other residents

What is Life Enrichment?

Life enrichment will be built into each resident’s care plan. Purpose-driven activities, life experience stations and encouragement to participate help residents, who we call neighbors, maintain a sense of self-worth. We celebrate each small success while focusing on strengths rather than limitations.

Life Enrichment is a part of daily routine for our neighbors. We schedule activities for overall wellness such a morning hydration to help residents stay hydrated throughout the day, and hand massages in the afternoon to create a calm, quiet time before our evening rituals.

Music therapy is an activity that helps our neighbors communicate and feel joy. From live entertainers to individual playlists, music helps our residents socially connect to our staff, other residents, and their younger selves.

We meet with our memory care neighbors and their loved ones to understand each resident’s likes and dislikes, interests, and hobbies.
