Meadow Mills is a purpose-built memory care community located in Hamden, Connecticut. Hamden is known as “the land of the sleeping giant” for its picturesque mountaintop resembling a large man lying in repose. Meadow Mills is nestled between Dixwell and Whitney avenues on a tree-lined street and located directly across from the Whitney Center. It’s easily accessible to Route 15 and I-95 and just a few miles from Yale-New Haven Hospital’s St. Raphael Campus.
Our Community consists of four neighborhoods of 15 residents who live in private furnished rooms with private bathrooms. This allows for a peaceful environment for neighbors to enjoy. Each neighborhood provides a home environment, including the living spaces and dining experience.
Our beautiful outdoor area, which is lush with gardens, is private and secure. It was designed so each walking path leads back to the community, so a resident may never get lost. Residents and family members enjoy our relaxing walking paths, gazebos, and outdoor patio. They also enjoy our free-range chickens and raised gardens where residents plant and maintain herbs and vegetables.
Our specialized Life Experiences department was created to deliver activities based on each resident’s cognitive and physical abilities. Each experience or activity is specifically chosen to connect and relate to our residents. We also have a unique sensory cart that provides sensory stimulation to our residents to help decrease agitation and restlessness. Our sensory cart improves the quality of life for each of our residents, who benefit from one-on-one interaction when they are unable to participate in group activities.
Holidays and special days are celebrated and individualized gifts are given to each resident. Caring for dementia patients isn’t an afterthought; it’s our mission. Each resident is surrounded by an environment that is tailored to the unique challenges of memory loss, creating a special environment for our residents. At Meadow Mills, we help our residents thrive in cozy, familiar environments.
Following the guidelines of the National Alzheimer’s Association, we provide a nutritious and well-rounded dining experience that garnered a 2017 Pinnacle Customer Experience Award for outstanding food and dining services. In the spring and summer, herbs and vegetables grown by our residents are an addition to our meals.
Dining is in an intimate setting, with just 15 people in each neighborhood dining room. Meals are served by our friendly staff in small courses. Table linens and linen napkins are used during lunch and dinner.

Contact PLCThe Along the Journey program was created for residents with memory impairments. Working with the family, we get to know each resident’s rich history as well as their current daily life. This translates into a seamless transition upon move-in and a continued partnership throughout the resident’s ongoing journey.
